What the DOT physical actually covers.
The exam is set by federal standards. Knowing what's coming helps you walk in prepared and walk out with your card.
Vision Screening
You'll need to meet the federal standard of 20/40 vision in each eye, with or without corrective lenses, plus 70-degree field of vision. Bring your glasses or contacts if you wear them.
Hearing Screening
You must be able to hear a forced whisper at 5 feet in at least one ear, with or without a hearing aid. If you wear hearing aids, bring them and any documentation.
Blood Pressure & Pulse
BP under 140/90 is needed for a 2-year certificate. Higher readings may result in a shorter card or a return visit after follow-up. Bring your home BP log if you've been monitoring.
Urinalysis
A simple urine sample checks for protein, blood, and glucose. It is not a drug screen. If your employer requires drug testing, that's a separate visit.
Medical & Driving History
You'll fill out a federal health history form covering current medications, surgeries, sleep apnea, diabetes, cardiovascular conditions, and other items the FMCSA requires examiners to review.
Physical Exam
A thorough head-to-toe exam — heart, lungs, abdomen, joints, reflexes, neurological function. Dr. Saur reviews your full picture to determine the appropriate certificate length.
PREPARATION TIP
Conditions that can shorten or delay your card.
Diabetes, sleep apnea, heart disease, and uncontrolled high blood pressure can require additional documentation from a specialist (cardiologist, sleep doctor, etc.) before a full 2-year certificate can be issued. If you have any of these, bring your specialist's records to your appointment so we can issue the longest card you qualify for.

If you pass, Dr. Saur issues your Medical Examiner Certificate (DOT card) on the spot. The exam result is also uploaded to the FMCSA National Registry. You leave with the card and can keep driving.
