Sports Physicals
Pre-participation exams for student athletes from elementary through college. We check height, weight, blood pressure, vision, heart, lungs, joints, and reflexes — and complete the form so your athlete is cleared to play.
For Athletes Ages 5–CollegeMHSAA Forms
We complete the official Michigan High School Athletic Association pre-participation physical form — the one your school's athletic department requires. Bring it filled out as far as you can, and we'll handle the rest.
MHSAA ApprovedSchool Physicals
Annual physical exams required for school enrollment, kindergarten entry, or general student health. Includes growth tracking, vision and hearing screening, immunization review, and any required form completion.
For All Grade LevelsCamp Physicals
Required physicals for summer camps, scouting, and overnight programs. We follow the camp's specific form and address any medication, allergy, or activity restriction questions they need answered.
For Summer & Year-Round CampsIn, examined, signed off — usually under an hour.

Walk in & check in
Stop by 624 Charlevoix Avenue during open hours. Bring the school or league's physical form, your insurance card, and a photo ID. Filling out the parent/history portions of the form before you arrive helps us move quickly.

Physical exam
A provider performs the exam — vital signs, height and weight, vision, heart and lung check, joints, reflexes, and a brief health history review. We address any clearance concerns or activity restrictions on the spot.

The provider completes and signs the form during the visit. You leave with everything the school, league, or camp requires — no follow-up trip, no waiting on paperwork.
