Don't have a printer? No problem — show up a few minutes early and our front desk will give you the packet on a clipboard when you check in. Either way, we'll get you back to a room.

WHAT TO BRING

Six things to grab before you head over.

Doesn't have to be perfect. Bring what you can — we'll fill in the rest at check-in.

Photo ID

A driver's license is fine. State ID, passport, or military ID also work.

Insurance Card

The physical card or a clear photo of the front and back.

Completed Patient Packet

If you downloaded and filled it out at home. If not, no problem — we have copies.

List of Current Medications

Names and dosages of everything you take, including over-the-counter and supplements.

List of Allergies

Medication allergies, food allergies, environmental allergies, and your reaction to each.

Form of Payment

For copays, deductibles, or self-pay visits. We accept most major cards.

WHAT TO EXPECT

Your first visit, from the door to the goodbye.

We've designed every part of the visit to feel calm, unhurried, and personal — from the front desk to the exam room.

Walking in

You'll be greeted by a real person — not a kiosk. Our front desk team checks you in, takes your insurance and ID, and gets you a clipboard with anything we still need. The waiting area is quiet and comfortable.

ii.

The visit itself

A medical assistant takes vitals and asks a few questions, then your provider comes in. They'll listen — really listen — to why you're here, what's been on your mind, and what you'd like to address. There's no shot clock.

iii.

Heading home

Before you leave, you'll have a clear picture of what was done, what's next, and how to reach us if anything comes up. Lab work or imaging done on-site means fewer follow-up trips elsewhere.
